A treatment plan is a personalised roadmap for your mental health care. It outlines your goals, recommended therapies or interventions, and how your care team will support you — step by step. It’s flexible, evidence-based, and designed around your needs, preferences, and pace.

 

Who are Treatment Plans for?

Treatment plans are helpful for anyone starting or continuing mental health care. You may benefit if:

Treatment plans are co-created — we develop them with you, not for you. They support choice, clarity, and consistency across your care.

 

What does a Treatment Plan involve?

  • Identification of primary concerns (e.g. anxiety, low mood, trauma)

  • Clarification of diagnosis or working formulation (if helpful)

  • Discussion of treatment options (e.g. therapy, medication, coaching)

  • Timeline for sessions, reviews, and progress checkpoints

  • Support with decision-making, especially for complex needs

  • Optional input from family, GP, or care team (with your consent)

  • Plan tailored to identity, sensory needs, cultural or gender context

  • Regular review and adjustments as things change

  • Option to include lifestyle, work, or relationship support

  • Available as a written summary or shared verbal framework

  • Flexible, trauma-informed, and neurodiversity-affirming
