Who is Complex Case Management for?

This approach is designed for individuals whose mental health needs don’t fit neatly into one diagnosis or service. You may benefit from complex case management if:

  • You’ve had multiple mental health diagnoses or previous treatments

  • You live with co-occurring physical health conditions, neurodivergence, or trauma

  • You’ve had hospital admissions or crisis episodes

  • You’ve found traditional therapy or medication alone insufficient

  • Your care needs span multiple areas (e.g. psychiatry, therapy, social support)

It’s especially helpful for people who’ve struggled to access consistent or coordinated care, or who feel they “fall through the cracks” of traditional services. We support adults with functional needs, risk factors, or overlapping challenges like trauma, ADHD, autism, and personality-related difficulties.

What does Complex Case Management involve?

  • A dedicated lead clinician overseeing care

  • Comprehensive assessment across psychiatric, psychological, and social domains

  • Collaborative care planning with the client at the centre

  • Regular multidisciplinary reviews (psychiatry, psychology, nursing, etc.)

  • Crisis and safety planning

  • Medication review and management

  • Therapy tailored to complexity (e.g. trauma-focused, stabilisation, DBT-informed)

  • Coordination with GPs, social care, or other services

  • Support with housing, benefits, and daily living needs

  • Carer and family involvement (with consent)

  • Trauma-informed, neurodiversity-affirming approach

  • Advocacy during transitions or system navigation

  • Online or in-person flexibility

  • Long-term relational consistency

  • Emphasis on autonomy and recovery
